Online Forms

NOTE: These forms can be signed electronically and submitted online

For courses being taught at the high school, Dual Enrollment students complete the following form:

Parent Consent Form (Only needs to be completed once for the duration of your high school years)

For courses being taught at GWC campus/online, Concurrent Enrollment students complete the following forms:

CE Release Form (Needs to be completed each semester)

Summer 2025 (current form)
Fall 2025 (new form)
AND
Parent Consent Form (Only needs to be completed once while taking courses at GWC)

PDF Forms

(To be used only in situations where the digital form is not accessible - must be printed, signed and submitted via email or in person)

Only needs to be completed once while student is taking courses at GWC while in High School.

Please send the signed and completed release forms to us at dualenrollment@gwc.cccd.edu as a PDF attachment or a high-quality photo. Students can also return them in person to the GWC Admissions and Records Office. GWC staff will process the release forms and email the student once they have permission to register.
