- Dual Enrollment Students Complete the following form:
- Parent Consent Form (Only needs to be completed once for the duration of your high school years)
- Courses being taught at the high school
- Concurrent Enrollment Students Complete the following forms:
- Concurrent Enrollment Release Form (Needs to be completed each semester)
- Parent Consent Form (Only needs to be completed once while taking courses at GWC)
- Courses being taught at GWC campus/online
PDF Forms
(must be printed, signed and submitted via email or in person)
- Only needs to be completed once while student is taking courses at GWC while in High School.
- Needs to be completed each semester
Please send the signed and completed release forms to us at dualenrollment@gwc.cccd.edu as a PDF attachment or a high-quality photo. Students can also return them in person to the GWC Admissions and Records Office. GWC staff will process the release forms and email the student once they have permission to register.
